We ask all new customers to fill out our Terms and Agreement Conditions Form before using our facility. Please review this form online and bring it with you, signed, upon your first visit to Misty Pines. If you are unable to print this form or if you forget it, we have them at our facility for your convience. Please either fax or e-mail a copy of your dog's vaccination records to us or bring them with you when you visit. See the bottom of this page for a list and description of required vaccinations. |
